When a patient or guarantor signs the Hospital or Ambulatory Surgical Center Financial Agreement form, they will be giving our group Assignment of Benefits and will only be responsible for their co-pay, co-insurance or deductible based on their insurance plan. This does not apply to SELF-FUNDED employer plans.
